Valoració i tractament de fisioteràpia en lesions totals o parcials del LCA en futbolistes professionals intervinguts quirúrgicament amb un empelt del semitendinós

Abstract

INTRODUCCIÓ: Les lesions del lligament creuat anterior (LCA) són una de les lesions lligamentoses més comunes entre els futbolistes professionals, afecten significativament la funcionalitat del genoll tenint repercussions físiques i psicològiques. Aquest treball profunditza en la comprensió del diagnòstic d'aquestes lesions i el seu tractament quirúrgic i la seva posterior rehabilitació. OBJECTIUS: L'objectiu principal és proporcionar una visió integral sobre la lesió del LCA i un pla de tractament prequirúrgic i postquirúrgic en futbolistes professionals, amb la finalitat de millorar les estratègies de prevenció, diagnòstic i tractament, centrant-se en la rehabilitació amb empelt del tendó semitendinós. RESULTATS: Les actuacions diagnòstiques que es porten a terme són proves específiques i RM, per tal de determinar la lesió. A partir d’aquestes actuacions diagnòstiques es podrà determinar si hi ha lesió o no i en cas d’haver-hi, si és de grau lleu, mig o greu. Actualment, no hi ha un protocol estandarditzat, tot i que el pla terapèutic segueix les fases de preoperatori, fase aguda, enfortiment muscular, entrenament funcional i return to play amb uns criteris per passar d’una fase a una altra. CONCLUSIONS: La valoració precisa i el tractament adequat són indispensables en la lesió de LCA. Es requereix una valoració integral del pacient i una col·laboració en l’àmbit multidisciplinari per realitzar un abordatge efectiu. Hi ha limitació quant a protocol estandarditzat i es necessita més evidencia tant quirúrgic com rehabilitador. Aquesta revisió posa de manifest la necessitat d'una avaluació i un tractament òptims en futbolistes professionals afectats per aquesta lesió, amb l'objectiu de facilitar el seu retorn a la pràctica esportiva en les millors condicions possibles. INTRODUCTION: Anterior cruciate ligament (LCA) injuries are one of the most common ligament injuries among professional footballers, significantly affecting knee functionality with physical and psychological repercussions. This work deepens the understanding of the diagnosis of these lesions and their surgical treatment and subsequent rehabilitation. OBJECTIVES: The main objective is to provide a comprehensive vision of the LCA injury and its pre-surgical and post-surgical treatment plan in professional footballers, to improve prevention, diagnosis, and treatment strategies, focusing on semi-tendinous tendon graft rehabilitation. RESULTS: The diagnostic actions that will be carried out will be specific tests and RM, to determine the injury. From these diagnostic actions, it will be possible to determine whether there is an injury or not and whether it is mild, medium, or serious. Currently, there is no standardized protocol, although the therapeutic plan follows the phases of the preoperative, acute phase, muscle strengthening, functional training, and return to play with criteria for progressing from one phase to the next. CONCLUSIONS: Precise assessment and proper treatment are indispensable in LCA injury. A comprehensive valuation of the patient and multidisciplinary collaboration is required for an effective approach. There are limitations in terms of standardized protocol and more evidence is needed for both surgical and rehabilitation wise. This review highlights the need for optimal evaluation and treatment in professional footballers affected by this injury, in order to facilitate their return to sports in the best possible conditions.
