Avaluació i abordatge fisioterapèutic de la motricitat grossa en infants amb Paràlisi Cerebral espàstica amb hemiplègia

Abstract

INTRODUCCIÓ: La paràlisi cerebral infantil (PCI) espàstica amb hemiplègia és un conjunt d’alteracions en els sistemes múscul-esquelètic, neuromuscular i sensorial que causen problemes en el moviment, en l’equilibri i fins i tot en la postura. La presència d’hemiplègia, manifesta un costat del cos amb més afectació, amb restriccions i amb moviments compensatoris, essent les extremitats superiors la zona més compromesa segons l’evidència trobada. OBJECTIUS: Diagnosticar i elaborar un pla de tractament per millorar les habilitats manuals d’infants amb PC espàstica amb hemiplègia. RESULTATS: Es realitza un procediment diagnòstic detallat amb una anamnesi, una entrevista conjunta a l’infant i a la família, i una exploració clínica. A partir d’aquí, es classifiquen tres grups d’infants amb diferents graus d’afectació motriu en la seva extremitat superior plègica. Per a la seva valoració s’utilitzaran les escales GMFCS, l’escala d’Ashworth i l’escala MACS. El pla terapèutic proposa una part de rehabilitació conjunta de fisioteràpia convencional, a més de la intervenció en realitat virtual adaptada per a cada grup. CONCLUSIONS: L’evidència descriu una àmplia diversitat de tractaments per aquest trastorn, tant mèdics com de rehabilitació en fisioteràpia neurològica. És important aplicar un pla de rehabilitació específic a partir d’un bon diagnòstic i valoracions inicials. Amb l’ajuda de la realitat virtual, molts infants amb PC i hemiplègia es veuen beneficiats i motivats en treballar per a obtenir millores en la funcionalitat de la seva extremitat superior. INTRODUCTION: Spastic cerebral palsy (CP) with hemiplegia is a set of disorders in the musculoskeletal, neuromuscular and sensory systems that cause problems in movement, balance and even posture. The presence of hemiplegia, manifests one side of the body with more affectation, with restrictions and compensatory movements, the upper limbs being the most compromised area according to the evidence found. OBJECTIVES: To diagnose and develop a treatment plan to improve manual skills in minors with SCP with hemiplegia. RESULTS: A detailed diagnostic procedure is carried out with an anamnesis, a joint interview with the child and family and a clinical examination. These three groups of children are classified with different degrees of motor impairment in their plegic upper limb. The GMFCS scales, the Ashworth scale and the MACS scale will be used for their assessment. The therapeutic plan proposes a joint rehabilitation part of conventional physiotherapy, in addition to the intervention with virtual reality adapted for each group. CONCLUSIONS: The evidence describes a wide diversity of treatments for this disorder, both medical and neurological physiotherapy rehabilitation. It is important to implement a specific rehabilitation plan based on a good diagnosis and initial assessments. With the help of virtual reality, many children with SCP and hemiplegia benefit and are motivated to work towards improvements in the functionality of their upper limb.
