Dolor lumbar crònic en la síndrome de l’intestí irritable (SII): diagnòstic, avaluació i pautes de tractament

Abstract

INTRODUCCIÓ: La síndrome de l’intestí irritable (SII) és un trastorn gastrointestinal crònic comú que cursa amb dolor abdominal recurrent, malestar i alteració de l’hàbit intestinal. És un trastorn orgànic amb patogènesi multifactorial i en són característics la hipersensibilitat visceral i els símptomes psicològics. Freqüentment, els pacients amb SII es queixen de dolor en regions del cos llunyanes de l’intestí, fet que indica un procés de sensibilització implicat. OBJECTIUS: Ampliar els nostres coneixements sobre la SII per identificar els pacients amb dolor lumbar degut a aquest trastorn i així realitzar el millor diagnòstic, avaluació i tractament possible. RESULTATS – PLA D’ACTUACIÓ: Es proposa una anamnesi detallada, una exploració sistemàtica i una valoració minuciosa per identificar els pacients amb dolor lumbar crònic i SII. Un cop identificats, es classifica els pacients en dos grups segons l’etiologia i la simptomatologia. A continuació, s’elabora un pla de tractament general i comú, així com plans multidisciplinaris especialitzats dirigits a cada grup. CONCLUSIONS: El pla diagnòstic i terapèutic elaborat es podria aplicar a la pràctica clínica en pacients adults amb dolor lumbar crònic d’origen visceral, concretament de la SII, abordant el trastorn des de diferents àmbits i amb un treball multidisciplinar. INTRODUCTION: Irritable bowel syndrome (IBS) is a common chronic gastrointestinal disorder that presents recurrent abdominal pain, discomfort and altered bowel habits. It is an organic disorder with a multifactorial pathogenesis and its characteristics include visceral hypersensitivity and psychological symptoms. Frequently, patients with IBS complain of pain in parts of the body distant from the intestine, indicating a sensitization process involved. OBJECTIVES: To expand our knowledge of IBS to identify patients with resulting low back pain due to this disorder and thus perform the best diagnosis, evaluation and treatment possible. RESULTS – ACTION PLAN: A detailed medical history taking, a systematic physical examination and a meticulous assessment to identify patients with chronic low back pain and IBS has been proposed. Once identified, the patients were classified into two groups according to etiology and symptomatology. Then a general and common treatment plan was developed, as well as specialized multidisciplinary plans aimed at each group. CONCLUSIONS: The diagnostic and therapeutic plans developed could be applied to clinical practice in adult patients with chronic low back pain of visceral origin, specifically IBS, approaching the disorder with different treatments within a multidisciplinary work.
